Correction to Phone Number- The phone number for Metropolitan Life in the July 30th GM Shop Report and the August issue of the Local 95 Union News was incorrect. The number should be 1-888-543-3461 (not an 800). If you are saving these numbers, please make a correction.

Information Regarding U/C for Upcoming Weeks

Due to the upcoming lay off for the weeks of August 18th & 25th
and September 1st, employees will be calling in for Wisconsin Unemployment and collecting Automated SUB. Some employees may be put on indefinite layoff and will not be returned on Sept. 8th, and as such, will not receive Holiday Pay for the September 1st
Labor Day Holiday. For those employees returning to work on September 8th, you will need to claim 10 hours Holiday Pay with the State of Wisconsin Unemployment for week ending September 6th.

Note:  The effective date for the Cost of living adjustment will be September 1, 2008. This new rate will affect your holiday pay and is reflected in the amounts below.

This is a good time to save a payroll statement so you can report the correct amount of Holiday Pay. On the upper right hand corner of your pay statement is your RATE and COLA. If you add Rate plus COLA times 10 hours you should arrive at your Holiday Pay. (i.e. Rate 28.71 plus COLA of 1.08 = 29.79 X 10 = $297.90; Rate 29.28 plus COLA of 1.08 = 30.36 X 10 = $303.60.) Please note: There are five different pay scales between Assembly and Skilled Trades, so make sure you report yours correctly. You will have until Saturday, Sept. 13, 2008 to report this to Wisconsin UC. If in doubt, upon return to work Monday, Sept. 8th, check with your Supervisor to obtain the correct amount to claim.

UAW GM and Delphi Legal Services Plan Coverage

As you may be aware, UAW members eligible for the Legal Services Plan must pay imputed income.

Previously, in certain situations some participants had an increased tax liability.  For example, if both spouses work at either GM or Delphi, or have coverage as a retiree and surviving spouse from either company, the participants may be charged two imputed income amounts.

To avoid this increased liability and if the above stated circumstances exist, one eligible participant of the Plan may now declare coverage for both eligible parties for tax purposes.  This will result in both parties enjoying the same level of legal service coverage and only one member being charged with imputed income.

To provide notice of dual coverage or to delete or reinstate coverage, the member must call (800) 521-7818 Ex. 517 and indicate they are calling regarding dual Plan coverage.  Eligible participants' names, social security numbers, locations or employment and local union numbers should be available at the time of the call.

Notice of dual coverage from the tax paying party must be received no later than Friday, September 26, 2008 and will become effective November 3, 2008.

There is no need for further action in this regard if the Legal Services Plan was previously notified.

CANCEL YOUR INELIGIBLE DEPENDENT COVERAGE

UAW Local 95 has been notified by General Motors that all enrollees in the General Motors Health Care Program may have to verify dependent eligibility.  We have been told that the deadline for canceling ineligible dependents is August 20, 2008

You should be receiving a letter from General Motors regarding dependent coverage with all your dependents listed.  Please review the list and check to make sure all dependents are eligible for coverage.  If all of your dependents remain eligible for coverage, you do not need to do anything at this time.  Coverage will continue for all of the dependents named.  If you find that a dependent is no longer eligible, you will need to cancel their coverage.

An explanation regarding who is eligible will be included in the letter that you receive.  To cancel coverage for an ineligible dependent, call the GM Benefit and Service Center at 1-800-489-4646. You will need your Social Security Number or Fidelity Customer ID and Personal Identification Number (PIN).
1.
From the main menu, say “Health and Insurance.”
2.
   Say “Dependent Verification.”
3.
Indicate “Eligible or Ineligible” for each of your dependents.

We were told that later in the year, you may be asked to provide documentation for those remaining eligible dependents.  Dependents found to be ineligible later may result in a financial penalty to you.

If you have any questions or need further assistance, contact the UAW/GM Benefit Representatives at 755-5126 at the GM Plant.  Benefit representatives are available at the Union Hall on Mondays, Tuesdays and Thursdays from 1:00-3:00 p.m. to also assist you.

Benefit Representative Hours During Lay Off:

For the 1st and 3rd week of lay off, the weeks of August 18th and September 1st , there will be Benefit Reps. in the Plant on their regular 1st shift schedule; and a Benefit Rep. at the Union Hall from 8 a.m. until 6 p.m., Monday - Thursday. The week of August 25th will be a Cold Shutdown and there will be NO Benefit Reps. available in the Plant or the Union Hall.
